HEALTH CARE PLANS changes: who to notify

If you change your coverage from the traditional fee-for-service program, you should notify all individuals who may be involved in helping you obtain medical services. Give them the name of your new plan, including the primary care physician you have selected. Also, you may want to include a list of any other providers who have contracted with your plan to provide covered services, such as hospitals, skilled nursing facilities, pharmacists, etc.

Here's a short list of some of the people you may want or need to contact:


  • friend(s) or relative(s), if you sometimes rely on them to help manage your health care and insurance needs
  • housing manager or service coordinator, if you are a resident of a government or community sponsored subsidized housing unit
  • administrator, director of health care services, or social worker if you are a resident of a continuing care retirement community, a personal care home, or nursing facility
  • Social Security Administration office, if you disenroll from a managed care plan and want to return to the standard Medicare fee-for-service coverage
